CDL/DOT Physicals

The Department of Transportation (DOT) requires health exams for commercial motor vehicle (CMV) operators driving vehicles that weigh five tons or more. Dr. Panza is a Certified Medical Examiner through the Federal Motor Carrier Safety Administration (FMCSA).

  • Please bring your driver’s license and a copy of current medications. If possible please text your drivers’ license to 360.824.3404 prior appointment and confirm if address on ID is current.

  • Please complete your DOT forms prior to your visit; Click here; DOT FORMS (Print or Email to clinic).

  • Fee: $190 ($180 cash). We do have billing accounts with some local employers. These cannot be billed to insurance.

  • Other documentation you may need

    • See the FMSCA form page for pertinent forms you may need.

    • Sleep apnea: Please bring CPAP usage report to your appointment if you have sleep apnea

    • Diabetes: A1c lab work within last 3 months. If insulin treated - Insulin FMSCA Forms

    • Vision impairments: If you have a disqualifying vision condition your ophthalmologist will need to fill out a Vision Waiver for your CDL exam

    • Controlled Prescriptions: If you are taking a controlled substance (Adderall, Xanax, Opiates) you will need your prescribing provider to clear you with a FMSCA Medication Waiver
